ADHA Annual Conference Needs Assessment Drawing

(“OFFICIAL TERMS AND CONDITIONS”)

The “ADHA Annual Conference Needs Assessment Drawing” (the “Drawing”) is sponsored by the American Dental Hygienists’ Association (the “Sponsor”). This Drawing is governed by these Official Terms and Conditions. By participating in the Drawing, each entrant agrees to abide by these Official Terms and Conditions, including all eligibility requirements, and understands that the results of the Drawing, as determined by the Sponsor and its agents, are final in all respects. The Drawing is subject to all federal, state, and local laws and regulations and is void where prohibited by law.

  1. ELIGIBILITY: This Drawing is open only to legal residents of the United States who are at least eighteen (18) years of age or older (or the legal age of majority in their state of residence, whichever is greater) at the time of entry. Entrants must be a currently licensed or credentialed dental hygienist or an enrolled dental hygiene student. Employees, officers, and directors (including immediate family members and members of the same household, whether or not related) of the Sponsor and any companies associated with the Drawing are not eligible to participate or win. Eligibility is contingent upon compliance with these Official Terms and Conditions and fulfilling all requirements set forth herein. Void where prohibited or restricted by law, rule, or regulation.
  2. HOW TO ENTER: There is no entry fee, and no purchase or membership is necessary to enter or win this Drawing. The Drawing entry period begins at 9:00 a.m. CT on February 11, 2026, and ends at 11:59 p.m. CT on February 27, 2026 (the “Entry Period”). Entrants may enter by completing the ADHA Annual Conference Needs Assessment Survey and then submitting their information through the separate Drawing entry form. Completion of the survey is not required for entry. Alternative entry may be made by sending an email to [email protected] with the subject line “Conference Drawing Entry” and including the entrant’s full name, credentials, state of residence, and email address. Limit one (1) entry per person regardless of method of entry. All entries must be received during the Entry Period. Sponsor reserves the right to extend or shorten the Entry Period at its sole discretion.
  3. PRIZE DESCRIPTION:
    Grand Prize (1): One (1) complimentary registration to the ADHA26 Annual Conference in Fort Worth, Texas (September 26–28, 2026). Approximate retail value: $500+. The registration is non-transferable and may only be used by the winner. The registration may not be sold, assigned, or given to another individual. If the winner is unable to attend ADHA26, the prize will be forfeited with no cash alternative or substitution.
    Early Bird Prize (1): The first person to complete the survey will receive a $50 gift card, which will be delivered by mail or electronically by March 27, 2026.
    No cash alternative to any prize will be offered. Prizes are subject to availability, and the Sponsor reserves the right to substitute any prize with another of equivalent value without giving notice. ALL TAXES ARE THE SOLE RESPONSIBILITY OF THE WINNER. Prizes are awarded without warranty, express or implied, of any kind.
  4. WINNER SELECTION AND NOTIFICATION: One (1) Grand Prize winner will be selected at random from all eligible entries received during the Entry Period. The drawing will take place on or before March 27, 2026. The winner will be notified via the email address provided at the time of entry. If the winner cannot be contacted, does not respond within ten (10) calendar days of notification, or is found to be ineligible, the Sponsor reserves the right to forfeit the prize and select an alternate winner.
  5. CONDITIONS OF PARTICIPATION: No transfer, assignment, or substitution of any prize is permitted, except the Sponsor reserves the right to substitute a prize for an item of equal or greater value at its sole discretion. All federal, state, and local laws and regulations apply. Entrants agree to be bound by the terms of these Official Terms and Conditions and by the decisions of the Sponsor, which are final and binding on all matters pertaining to this Drawing. By entering and/or accepting the prize, entrants and winners agree to hold and indemnify the Sponsor and its subsidiaries, affiliates, directors, officers, and employees harmless for liability, damages, or claims for injury or loss to any person or property, relating to, in whole or in part, directly or indirectly, participation in this Drawing, the acceptance and/or subsequent use or misuse of the prize awarded, or any claims including but not limited to claims based on publicity rights, defamation, invasion of privacy, or copyright infringement. False or deceptive entries or acts will render the entrant ineligible. Sponsor, in its sole discretion, reserves the right to disqualify any entrant who commits or has committed any act, or is involved in any situation or occurrence, which the Sponsor deems likely to subject the Sponsor to ridicule, scandal, or contempt, or which reflects unfavorably upon the Sponsor in any way.
  6. DATA AND PRIVACY:  Entry information collected through the Drawing entry form is separate from survey responses and will not be connected to survey data. Entry information will be used solely for the purposes of administering the Drawing and, if the entrant has opted in, to send promotional communications about ADHA26.   7. CONDUCT: All entrants agree to be bound by these Official Terms and Conditions. The Sponsor, in its sole discretion, reserves the right to disqualify any person it finds to be tampering with the entry process or the operation of the Drawing, or who has otherwise violated these terms.
  8. LIMITATIONS OF LIABILITY: Sponsor is not responsible for lost or late entries, nor for electronic transmission errors resulting in omission, interruption, deletion, defect, delay in operations or transmission, theft or destruction or unauthorized access to or alterations of entry materials, or for technical, network, telephone equipment, electronic, computer, hardware, or software malfunctions or limitations of any kind, or inaccurate transmissions of or failure to receive entry information by Sponsor on account of technical problems or traffic congestion on the Internet or at any website, or any combination thereof. If for any reason the Drawing is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes that corrupt or affect the administration, security, fairness, integrity, or proper conduct of this Drawing, the Sponsor reserves the right at its sole discretion to disqualify any individual who tampers with the entry process and to cancel, terminate, modify, or suspend the Drawing. Sponsor reserves the right to select a winner from eligible entries received as of the termination date.
  9. GOVERNING LAW: This Drawing is governed by the laws of the State of Illinois, without regard to principles of conflict of laws. All cases and claims pertaining to this Drawing must be brought in a court of competent jurisdiction in Chicago, Illinois, without recourse to class-action suits.
  10. SEVERABILITY: If any provision of these Official Terms and Conditions is found to be invalid or unenforceable by a court of competent jurisdiction or appointed arbitrator, such determination shall in no way affect the validity or enforceability of any other provision herein.
