ADHA is dedicated to the future of the dental hygiene profession and wants to support your efforts as student advisors. A number of resources have been created to help you establish, maintain and enhance an ADHA student chapter.
ADHA Student Chapter Manual
The ADHA Student Chapter Manual will show you how to establish a student chapter and to enhance your student chapter activities and community connection. It also contains sample student chapter constitution and bylaws.

Student Advisor Guidelines
These guidelines contain more information on what is expected of the role of ADHA Student Chapter Advisor.
 ADHA Student Chapter Advisor Contact Form
All ADHA student chapters must have a student advisor. To be recognized as an ADHA student chapter, all student advisors must complete the contact form and submit to ADHA’s Division of Member Services – Student Relations.
ADHA Student Member Roster
Please complete this form and submit to ADHA’s Division of Member Services – Student Relations. ADHA will accept this roster with or without payment depending on if your program pays the ADHA student dues or if the students pay online individually.
Online Membership Sign-up Worksheet - New Members or Renewing Members
This form provides your students with step-by-step instructions on signing up for ADHA student membership online. Included are instructions for new and returning students who are either paying online individually or creating a temporary ADHA member ID number to submit to their student advisor.

ADHA is encouraging all student chapters to use the online sign-up format, however the hard copy application is still available.
