The ADHA Institute Scholarship Application is revised
annually by the Scholarship Committee. Thus, a revised version will
be available after January 1 of each year. If you have specific
questions before that date, please contact Institute staff at institute@adha.net.

| Scholarship Recipients:

All applicants will
be notified of the Scholarship Committee's decision by mail
prior to October 30th. |
When you have completed all of the student applicant
fields, please print the application and sign your name in the designated
places. Then, take a paper copy of the application (you can also
provide a copy of the pdf file on disk) to your faculty advisor,
program director, and financial aid office. Follow the rest of the
directions located on the application and mail the finished product
to the ADHA Institute.
